DHD#10 is hosting a COVID-19 Vaccine Pop-Up Clinic at the following location:

DATE: Wednesday, June 9
TIME: 11:00am-3:00pm
- Offering Pfizer (Age 12+), Moderna (Age 18+), and Johnson & Johnson (Age 18+)
- Students, families members, and community members welcome
- Vaccine is FREE – Bring driver’s license and insurance card if you have it
- No appointment needed. Just walk in
- Students age 12-15 MUST have parent/legal guardian present
- Students age 16-17 do not need parent/legal guardian present but must bring completed and signed registration and consent forms to appointment
