DHD#10 conducts investigations of reported illness connected with a licensed food service establishment in the district. Common foodborne illness symptoms consist of nausea, vomiting, diarrhea and on average can last 24 to 48 hours.
Please be prepared to provide the following information:
- Name, address and telephone number (you will remain anonymous to the restaurant)
- Name of food service establishment
- Location/address of the food service establishment (it must be within the ten (10) county district)
- Date and time of suspect meal
- Information about the foods you believe made you sick
- When your symptoms began and what they were
- Three day food history
Reporting Ill Employees
The food service license holder is responsible for reporting certain diagnoses and symptoms to the Department. Contact us immediately to report an employee who has been diagnosed with Jaundice (yellowing of the skin and/or eyes). Please call DHD#10 to report any of the following illnesses that your employee was diagnosed by a health practitioner:
Hepatitis A
Norovirus
Salmonella Typhi
Non-typhoidal Salmonella
Shigella
E. coli (EHEC or STEC)
