Breast & Cervical Cancer Screening and Navigation Program
Become a BCCCNP Provider to:
- Provide breast and cervical screening services and many diagnostic services at no charge to your low- and moderate-income women.
- Work with a BCCCNP agency, case management services can be provided to assist you in following women who have breast or cervical screening abnormalities.
- BCCCP women are eligible to apply for Medicaid coverage that provides all medical care for them through the entire course of their treatment.
- Receive reports on the care delivered to women in the program.

STD & HIV Testing & Counseling
Testing available for:
- Chlamydia,Gonorrhea, Syphilis, HIV,Hepatitis A,B and C
- Treatment and partner services available.
- Prevention counseling.
Low cost services on sliding fee scale.

Sexual Health Services
- Birth Control – broad range available, including IUD’s and Nexplanon
- Referrals for sterilization
- Preconception counseling
- Pregnancy testing and counseling
- Pap and HPV testing

We can bill insurance, low cost services available for uninsured, and no income requirements.

Women, Infants, & Children (WIC)
WIC gives families free healthy food on a WIC Bridge Card. It is a nutrition program that helps participants eat well and stay healthy. Some of the additional services available to WIC clients includes nutrition counseling, breastfeeding education and formula, and support.
Consider referring families who met the following qualifications:
- Women who are pregnant, breastfeeding, or who recently had a baby are eligible as well infants and children up to age 5. Must live in Michigan and meet income guidelines. Being enrolled in medicaid and/or SNAP automatically qualifies you for WIC.
Adoptive parents, fathers, grandparents, other family members, foster parents and other non-traditional custodial situations may be eligible to receive WIC benefits for infants /children in their care.

Children’s Special Health Care Services
Connect your clients to the Children’s Special Health Care Services Program available through District Health Department #10. This program that helps individuals (0-26 years old) that have special needs and health concerns to have improved health outcomes and an enhanced quality of life.
Children’s Special Health Care Service provides:
- Coverage and referral for specialty services based on the person’s health problems.
- Family centered services to support you in your role as primary caretaker of your child.
- Community-based services to help you care for your child at home and maintain normal routines.
- Culturally competent services which demonstrate awareness of cultural differences.
- Coordinated services to pull together the services of many different providers who work within different agencies
Who is eligible for CSHCS?
Qualifying Medical Condition
CSHCS covers more than 2,700 physical conditions when certain criteria are met. NOTE: CSHCS does not cover developmental, behavioral or intellectual conditions. Click here for a list of these diagnoses.
Age
Children must be under the age of 21. Persons 21 and older with cystic fibrosis or certain hereditary blood coagulation disorders commonly known as hemophilia may also qualify.
Citizenship
The client, or a parent of the minor client, or court appointed guardian must be one of the following:
- A US citizen, or
- A documented non-citizen who has been admitted for permanent residence, or
- A non-citizen legally admitted migrant farm worker (i.e., seasonal agricultural worker).
Residency
Must be a Michigan resident
Eligibility
- Is not based on income. Families with any income are eligible as long as the previous criteria are met.
There is a fee to join CSHCS. This fee is waived if the client has Medicaid, MIChild, a court-appointed guardian or lives in a foster home. The fee is determined with a sliding scale based upon family income and family size.
Healthy Futures
Healthy Futures helps to ensure every child has a healthy future, beginning at conception.
Program Benefits:
- Support from a registered nurse during pregnancy and throughout the first six months of your child’s life.
- After baby is born, a registered nurse can visit participants in their home to answer questions and connect participants with community resources.
- Participants will receive periodic newsletters in the mail with information on immunizations, safety, nutrition, and other issues affecting pregnancy and child development up to age 5.

Healthy Futures is available through a partnership of local health care providers, health departments, and Munson Medical Center.
Healthy Families of Northern Michigan (HFNM)
Healthy Families will share child growth and development information and activities with you, along with personalized, family-focused support in the comfort and convenience of your home. Your family support coach can share information on the resources available in your community.
Program Benefits:
- Will service families from prenatal time to the child’s 4th Birthday.
- Referrals need to be received during pregnancy or before the child is 3 months old. Prenatal referrals are preferred.
- This program is not income based but based on family support needs.
- To receive services, you must answer “True” to at least 1 of the 4 questions on the screener/referral form.

Maternal Infant Health Program (MIHP)
The Maternal Infant Health Program provides in-home support to pregnant women and infants who are Medicaid eligible. MIHP staff will help to make sure pregnant women receive proper prenatal care and infant care after delivery. A registered nurse, social work, and/or a registered dietitian will be at each visit and can connect your client to local resources, provide referrals, and answer questions.

Immunizations
Immunizing the community is a team effort. District Health Department #10 partners with health care providers in protecting the public from vaccine-preventable diseases. Our immunization nurses, clerks, and central scheduling staff are available to answer questions regarding vaccines, insurance coverage, the Vaccines for Children program, and many other topics related to immunizations.

- Immunization Action Coalition– vaccine Standing Orders for provider offices, vaccine administration tools, IAC Express e-mail newsletters, and helpful Ask the Experts answers to vaccine-related questions.
- Michigan Influenza Sentinel Provider Surveillance– Join MDHHS on the front lines of testing patients for influenza and help identify influenza cases and trends, as well as earn eligibility for free registration at MDHHS Fall Regional Immunization Conferences.
- Michigan Care Improvement Registry– is your link into Michigan’s immunization records. You can find MCIR vaccine inventory tip sheets, information on patient vaccine recall, vaccine waiver information and medical waiver forms, and much more.
- Children’s Hospital of Philadelphia– many easy-to-understand vaccine resources for parents and providers, as well as tear-off vaccine information sheets that can be ordered by provider offices.
