The HIV/AIDS Support Services Program provides services and support to enhance the daily life of persons infected with HIV and AIDS |
| GOAL: To provide support through client case management and advocacy, along with financial assistance for early medical intervention and supportive services. |
| ELIGIBILITY: |
HIV+ or diagnosis of AIDS
|
Family member/care giver of someone who is HIV+ or has been diagnosed with AIDS
|
Live in the DHD#10 service area (Crawford, Kalkaska, Lake, Manistee, Mason, Mecosta, Missaukee, Newaygo, Oceana, or Wexford County)

To access emergency financial assistance funds, you must demonstrate a financial need.

AVAILABLE SERVICES: |
| Case Management: |
A case manager can help you develop a plan of action for the management of your HIV; help you reach your goals, and maneuver through available community service. (Available only in Lake, Manistee, Mason, Mecosta, Newaygo, and Oceana Counties)

| Client Advocacy: |
Provides short term help with maneuvering through community agencies or immediate one-time financial assistance.
|
| Emergency Financial Assistance Funds: |
Funding available to help with short-term needs related to their Care Plan including transportation reimbursement, food, medications, and outpatient medical care. (Must meet financial guidelines to access)

| Housing Assistance: |
Provides help in locating housing, rental assistance, and short-term rent/mortgage/utility assistance. Services are available through federal Housing Opportunities for Persons with HIV/AIDS (HOPWA) funding.
